Doctor's Mistakes
“This isn’t an issue of incompetent people making stupid mistakes,” he says. “It’s many average, decent people working in poorly designed systems. Most medical mistakes were accidents waiting to happen.”

It made me think that doctors should organize themselves on-line under a "patient care" umbrella. Wouldn't it be great if you went to your doctor and your entire treatment path (including notes from every doctor that you had seen along the way and results from every test that you have taken) were laid out in front of you and your doctor on a computer screen? It would allow for a more careful review.

That would still require doctors actually do the required work to input the information. Which trust me, is not a given.
